Name, address, phone number, email address, date of birth, medical history.

Purpose of collecting data

I will be collecting data to provide and manage care, including taking notes during sessions, maintaining contact details and managing administrative records like appointments and billing. 

How will my data be stored?

 In May 2018 the Data Protection Act was replaced by the General Data Protection Regulations (GDPR). The changes to the Data Protection Act are aimed at ensuring that your personal, confidential and sometimes sensitive data, is held privately and securely. Your data will be stored in password protected files.
